The forms needed for requesting a records transfer can be obtained below. In either of the formats you can type directly into the computer. If you want to keep an electronic copy for your records, use the MSWord format. Print out the completed form, obtain the necessary signature, and send it to the University Records Archivist, 101 Conrad Hall.

If you have any difficulty with the process or you are still not able to view or use a form properly, call the University Archives at 355-2330.

If you would like to save an electronic version of your form you will need to use the WORD format.

For permanent or temporary return of records please fill out the Reference Request Form.

You will need Adobe Acrobat Reader to view the PDF Format. You can download this free program by clicking on the icon below and following the installation instructions provided.
